### Healthy Longevity Plan You want a clear health assessment and a plan you can act on. Format: Focused assessment and a one-hour clinic visit What you receive: Blood tests, a physical exam and a one-hour visit that includes a longevity physician consultation and your personalized written 90-day plan. Additional tests and ongoing care are separate. Healthy Longevity Plan

Healthy Longevity Plan

Blood tests, a physical exam and a one-hour visit that includes a longevity physician consultation and your personalized written 90-day plan.

HLC Baseline

One coordinated day / approximately 3–6 active hours. Assessment across blood and metabolic health, cardiovascular health, body composition and physical function, with cognitive assessment where clinically relevant. Physician synthesis and written priorities give you a clear next step.

Roadmap and Signature

HLC Roadmap provides twelve months of implementation, scheduled physician reviews, selected reassessment and an evolving plan. HLC Signature adds greater diagnostic depth, expanded evidence review, more frequent senior clinical input and priority access, informed by HLC’s scientific ecosystem. You can discuss either relationship directly.

Continuing is a decision too

A consultation can lead to further care, referral, monitoring, or no additional service for now. Returning clients use the separate client platform for their agreed follow-up.
