Begin with your priorities and an agreed plan. Put the recommendations into practice, review what changes and update the next steps with your physician. Your review schedule, included services and support are agreed before you begin.
You can discuss Roadmap directly or after an assessment. The team first reviews whether the information needed to begin is available. Diagnostics, procedures and other services are included only where your agreement specifies them.
Your schedule, included reviews, responsibilities and fees are agreed privately before the twelve-month relationship begins. The time required depends on your assessment and implementation needs. This relationship is separate from the 90-day action roadmap within HLP.
